About the role
Malmsbury Youth Justice Precinct is a youth custodial facility that accommodates young people. At Malmsbury, cohealth aims to deliver an efficient service within the custodial environment to young people.
The Clinical Care Coordinator supports the delivery of high-quality healthcare services for young people in custody by coordinating care, advocating for patient needs, and ensuring a holistic approach to health and reintegration.
The Clinical Care Coordinator works collaboratively with external and internal stakeholders and care providers to ensure young people receive timely interventions, appropriate referrals, and continuity of care post-release.

Your duties include:
- Coordinate care planning and health service access across medical, allied health and mental health teams for young people in custody
- Ensure smooth transitions for young people leaving custody by facilitating referrals and healthcare handovers.
- Maintain productive multi-disciplinary working relationships, effectively dealing with challenging behaviours and the resolution of conflicts

Our ideal candidate:
- Current AHPRA registration (AHPRA) as a Registered Nurse
- Knowledge of the Youth Justice System and understanding of the restrictions, limitations and considerations health care delivery within this system
- High-level stakeholder engagement skills, including working with external healthcare providers and Youth Justice leadership.
- Strong belief and personal alignment in cohealth Values and Mission
