About the Role
We are seeking a passionate and client-focused Case Manager to join our Housing & Homelessness Services team in Goulburn. This provides outcome-driven support to individuals and families experiencing complex challenges, including housing insecurity and crisis situations.
You will work collaboratively with clients to understand their needs, develop and implement case plans, reduce risks, and help them work toward positive and sustainable outcomes. Some rostered on-call duties, including occasional weekends, may apply.
What You'll Be Doing
- Build culturally respectful relationships and develop cultural case plans for Aboriginal clients.
- Maintain strong relationships with community partners to ensure integrated service delivery.
- Conduct client interviews, needs assessments, home visits, referrals, and advocacy.
- Develop, implement, and regularly review case plans for individuals, children, young people, and families.
- Support clients in setting realistic, strength-based goals and ensure their voice is central in all planning.
- Maintain accurate case notes, records, and reports in line with legislative and organisational requirements.
- Contribute to operational priorities and participate in professional development and monthly Sanctuary supervision.
- Participate in the after-hours on-call roster. (may be required)
What We're Looking For
- Minimum Diploma of Community Services or equivalent (Degree preferred).
- Experience supporting people experiencing homelessness.
- Experience working with individuals affected by Domestic Violence.
- Strong capability in client-centred case management.
- Experience working with Aboriginal people and communities is essential.
- Excellent communication, advocacy, conflict resolution, teamwork, and liaison skills.
- Strong administration and computer literacy.
- Understanding of the community services sector.
- Knowledge of trauma, crisis impacts, and child protection legislation.
