About the role

This position sits within the cohealth's Homelessness Services Cluster which aims to improve the wellbeing of people who use alcohol and other drugs and/or those experiencing homelessness.

The Homeless & Community Support: Group Engagement Worker aims to provide assistance to people who are living in inappropriate accommodation and support to people facing significant barriers to accessing services.

Your duties include:

  • Develop and implement client-directed, strengths based individual client care plans and service packages
  • Work collaboratively with clients to conduct comprehensive assessments and link individuals into appropriate ongoing community services
  • Support individuals to improve their daily living skills via a client centred goal directed approach
  • Conduct assertive outreach to locations where people experiencing an episode of homelessness are known to frequent to engage and link them into the services they require

Our ideal candidate:

  • A relevant tertiary qualification such as Social Work or equivalent experience in the housing, mental health, disability, or welfare sectors
  • Demonstrated experience in working with clients who have a history of homelessness and multiple and complex needs
  • Demonstrated experience in the development, delivery and evaluation of social support groups
  • Good time management and organisational skills, ability to work independently and within a multi-disciplinary team, which includes people with a lived experience
