The Opportunity
The Active Ageing Unit has an exciting opportunity for a Social Support Program Assistant to join the team. As our Social Support Program Assistant, you will be responsible for supporting the planning, delivery and evaluation of Social Support programs that align with the Commonwealth Home Support Programme (CHSP) and Home and Community Care for Younger People (HACCPYP) funded programs.
In addition to this, you will also provide support and assistance to eligible program participants to maximise their social connections and participation in Social Support programs.
Key Responsibilities
- Support the delivery of the Social Support program as guided by the Social Support Program Lead.
- Support the participation of all Social Support clients by providing social connectedness, activities and care in delivery of the program as guided by the Social Support Program Lead
- Assist clients as required to ensure their participation at Social Support that promotes their independence and reablement.
About You
To be considered for this role, you will have the following:
- Certificate 3 in Individual Support (Aged Care) preferred, or equivalent qualification with experience working within the care services industry.
- Experience in the delivery of programs that meet the needs of participants with physical or cognitive support needs.
- Current first aid and CPR certificate
- Current valid Victorian Drivers licence.
