Here's where you will make a difference

As our Clinical Coordinator, you'll play a vital role in coordinating care and services for our clients with Home Care Packages. You'll also support our Community Care Management team in various specialty areas such as gerontology, palliative care, hospital-to-home transitions, dementia support, and mental health. Your responsibilities will include advocating for clients, assessing their needs, and ensuring their well-being.

To be ideal for this role, you will bring the following:

• Professional qualifications and nurse registration in the ACT
• Knowledge of health, well-being, and support for older individuals
• Excellent negotiation, representation, and communication skills
• Strong time management, organizational capabilities and computer savvy with MS Office
• Outstanding relationship management skills with clients, medical professionals, service providers, families, and the community
• A commitment to good employer practices, including workplace health and safety, equal employment opportunities, and industrial democracy.
• Australian work rights. A valid driver's license and access to a registered, safe, and reliable vehicle with comprehensive car insurance for work
