About the role
The Community Health Nurse (CHN) will provide case coordination and service navigation to residents impacted by the public housing high rise redevelopment. The role ensures residents with complex needs or face barriers accessing care are supported to transition safely to new living environment. The role is part of a multidisciplinary team providing wrap around support to impacted residents.

Your duties include:
- Complete initial intakes to identify level of support needed for each resident being impacted by relocation
- Coordinate case management with relevant service providers
- Deliver support with a trauma informed approach and discuss cultural safety requirements where relevant
- Participate in relevant team meetings and staff development activities

Our ideal candidate:
- Tertiary qualifications in Nursing and eligibility to join the relevant professional body AHPRA
- Minimum 3 years' experience in community nursing position or equivalent
- Experience working with people being re-housed/resettlement
- Strong belief and personal alignment in cohealth Values and Mission
